<p>Wenn es um die Behandlung von Zwangsstörungen (OCD) geht, führt meist kein Weg an der Kognitiven Verhaltenstherapie (KVT) vorbei. Doch was ist mit EMDR, einer Methode, die eigentlich aus der Traumatherapie kommt? Eine spannende Studie von Marsden et al. (2018) hat genau diese beiden Methoden gegeneinander antreten lassen. Die Ergebnisse sind überraschend – und haben ein wichtiges statistisches Detail, das wir uns genau ansehen müssen.</p>
<p>Was ist eigentlich der „Goldstandard“? (ERP erklärt)<br />
Die weltweit am besten untersuchte und empfohlene Methode ist die KVT mit Exposition und Reaktionsmanagement (auf Englisch: Exposure and Response Prevention, kurz ERP). Das Prinzip klingt simpel, ist aber harte Arbeit: Patienten setzen sich gezielt den Situationen aus, die ihren Zwang auslösen (z. B. Türklinken anfassen), verzichten dann aber bewusst auf die sonst übliche Zwangshandlung (z. B. Händewaschen). Ziel ist es zu lernen, dass die Angst von alleine sinkt, ohne dass man das Ritual durchführt.</p>
<p><strong>Die Studie: EMDR vs. ERP</strong><br />
In der untersuchten randomisiert-kontrollierten Studie wurden 55 Teilnehmer mit Zwangsstörungen zufällig auf zwei Gruppen aufgeteilt:</p>
<p><strong>Die EMDR-Gruppe:</strong> Diese Patienten erhielten eine spezielle Anpassung des EMDR-Protokolls für Zwänge. Dabei wurden mit Hilfe der schnellen Augenbewegungen sowohl aktuelle Auslöser (die Zwangsgedanken) verarbeitet, dann Zukunftsszenarien als auch mögliche traumatische Ereignisse aus der Vergangenheit.</p>
<p><strong>Die KVT (ERP)-Gruppe:</strong> Diese Patienten erstellten eine „Angst-Hierarchie“ und übten systematisch, sich den Ängsten zu stellen (in vivo und in der Vorstellung), ohne die Zwangshandlungen auszuführen.</p>
<p>Die Ergebnisse: Ein statistisches Dilemma? Das Fazit der Forscher auf den ersten Blick: Beide Methoden funktionieren! Es gab statistisch keinen signifikanten Unterschied zwischen den Gruppen – beide zeigten Verbesserungen.</p>
<p>ABER – Der Blick auf die Zahlen (numerischer Trend): Hier wird es spannend: Wenn man sich die reinen Zahlen ansieht, hatte die KVT (ERP) die Nase vorn.</p>
<p>Der Effekt der Symptomreduktion war bei der KVT etwas stärker (Cohen’s d = -0.24 zugunsten der KVT).</p>
<p>Mehr Patienten in der KVT-Gruppe erreichten eine „klinisch signifikante Verbesserung“ (40,0 % bei KVT vs. 21,4 % bei EMDR).</p>
<p>Warum war das Ergebnis dann „nicht signifikant“? Das Problem war vermutlich vermutlichdie statistische Power. Mit nur 55 Teilnehmern war die Stichprobe zu klein, um diesen kleineren Vorteil der KVT statistisch zweifelsfrei zu beweisen. Man kann also vermuten: Hätte man 500 statt 55 Leute getestet, wäre die KVT wahrscheinlich auch statistisch als die überlegene Methode bestätigt worden.</p>
<p><strong>Fazit:</strong> EMDR erwies sich als machbare Alternative, besonders für Menschen, die ERP nicht machen können oder wollen. Dennoch bleibt die ERP (KVT) numerisch und basierend auf der breiten Studienlage der unangefochtene Platzhirsch.</p>

Dies liegt daran, dass der Wechsel von einem Zustand hoher Erregung (Angst) zu einem Zustand niedriger Erregung (Ruhe) schwer zu erreichen ist. Wenn man jedoch Angst als Aufregung umdeutet, bleibt man in einem Zustand hoher Erregung, was die Leistung in stressigen Situationen verbessern kann. Diese Reinterpretation kann zu einer positiveren Einstellung und gesteigerten Motivation führen, da Aufregung als produktiv und energiereich empfunden wird, während Angst oft als lähmend wirkt.<br />

<p>Zwangsstörungen stellen eine komplexe psychische Herausforderung dar, die nicht nur durch auffälliges Verhalten, sondern auch durch tief verwurzelte kognitive Prozesse geprägt ist. In diesem Zusammenhang erweist sich die <strong>Metakognitive Therapie</strong> als vielversprechender Ansatz, um Menschen mit Zwangsstörungen zu helfen, insbesondere indem sie sich auf die Entkräftung von Fehlüberzeugungen hinsichtlich der Mächtigkeit der eigenen Gedanken konzentriert.</p>
<p>Menschen mit Zwangsstörungen neigen dazu, ihre Gedanken als übermäßig bedeutsam und einflussreich zu bewerten. Der Gedanke, dass bestimmte Gedanken katastrophale Konsequenzen haben könnten, führt zu zwanghaften Verhaltensweisen. Metakognitive Therapie setzt hier an und erkennt die Notwendigkeit, diese übermäßige Bewertung zu durchbrechen. Es geht darum, den Betroffenen beizubringen, ihre Gedanken realistischer zu betrachten und zu verstehen, dass nicht jeder Gedanke die gleiche Macht hat.</p>
<p>Während die Verhaltenstherapie darauf abzielt, durch die Veränderung von Verhaltensweisen direkt Einfluss auf psychische Störungen zu nehmen, konzentriert sich die metakognitive Therapie auf die Metaebene der kognitiven Prozesse. Verhaltenstherapie kann beispielsweise darauf abzielen, zwanghaftes Handeln zu unterbrechen, während die metakognitive Therapie die zugrunde liegenden Denkmuster und Überzeugungen direkt angeht.</p>
<p>Im Video werden Einblicke in die metakognitive Therapie gegeben und eine neue Studie vorgestellt (Melchior et al., 2023), in der die Wirksamkeit der metakognitiven Therapie mit der Wirksamkeit der Verhaltenstherapie verglichen wurde.</p>

Aber wie kann die therapeutische Angst-Konfrontation noch effektiver gestaltet werden? Studien konnten zeigen, dass es bei der Behandlung von Angststörungen, langfristig effektiver ist, nicht immer wieder dieselbe Situation zu konfrontieren, sondern unterschiedliche angstauslösende Situationen zu meistern. Durch die Konfrontation mit verschiedenen Herausforderungen wird gelernt, dass die Angst nicht immer in jeder Situation berechtigt ist und man die Kontrolle über die Angst gewinnen kann. Eine einseitige Konfrontation mit einer bestimmten Situation kann jedoch beitragen, dass die Wahrscheinlichkeit einer Rückkehr der Angst (return of fear) vergleichsweise steigt. <p>So sinnvoll derartige Ratschläge in vielen Fällen auch sind, so problematisch können sie werden, wenn daraus die Überzeugung (Metakognition) erwächst, dass Gefühle IMMER Recht haben und UNTRÜGLICHE WEGWEISER sind. Nur weil sich der Konsum mancher Drogen fantastisch ANFÜHLT, heißt das noch lange nicht, dass sie mir auch langfristig gut tun. Ebenso kann das ausgeschlagene Bewerbungsgespräch bei der Traumfirma (weil man die Bewertungsangst nicht ertragen hat), langfristig zu Bedauern und Depressivität führen. Ziel der Psychotherapie ist also in vielen Fällen eine realistischere Sichtweise auf die eigenen Gefühle zu gewinnen. Gefühle sind gute Ratgeber, aber sie können sich auch mal irren&#8230;</p>
